INSURANCE:

I am pleased to accept insurance from nearly every insurance company offered by employers in Northeast Ohio*. I also retain Preferred Provider Status with these same companies meaning you can get the highest benefit level that your insurance offers when you work with me.

Should you not see your insurance company listed, please do not hesitate to call me directly, and I will help you to verify my provider panel membership. Sometimes, insurance companies have multiple branches and may be called by a different name. Other times, the website or company representative may list differently or may not be looking up my name correctly. To assist you in verifying insurance, please go to PATIENT FORMS - Insurance Paperwork and select Insurance Verification Script in .pdf or .doc format.

*QualChoice Members Please Note:
I have applied for membership in QualChoice Behavioral Health since 2004. However, they are not accepting new providers in the Cleveland area, despite my qualifications and specialty areas as well as service elements relatively unique to this area.

I am what is considered “out of network” with QualChoice. Thus, you can only use your Out of Network benefits which often involves a high deductible that must be met first. Once you meet your deductible, they will only cover 50% of the charges. Should you be a QualChoice member and wish to see me, please call me so we can discuss more economical alternatives that might better meet your needs.
